Scholarship Requirements
For Regular College/Ladderized Degree: For Incoming First Year College
- Thoroughly accomplished Application Form;
- One recent photograph (passport size);
- Application letter addressed to the Governor;
- Barangay Chairperson Certification that:
- Applicant is a bonafide resident of the barangay since birth/ for a period of not less than 1 year;
- Applicant’s parents are low income earners, stating the estimated annual income; and
- That applicant has no derogatory record/no past, current, or pending complaint filed against him or her.
- Certified photocopy of 1st and 2nd Semester Senior High Grades (present original);
- Certificate of Enrollment/ Statement of Account;
- Certificate of Good Moral Character from the school last attended;
- Certified True Copy of Birth Certificate (present original);
- Latest certified true copy of the applicant’s/ parents’/guardians’ BIR Income Tax Return, or if not available, a Barangay Certificate that the applicant’s family is a low income earner stating the estimated annual income;
- Certificate of No Scholarship Grant/Scholarship Application from the school where the applicant is enrolled;
- Applicant’s or applicant’s parents’ voter’s ID or voter’s certificate;
- Sketch Map of applicant’s home address;
- Documents commending the applicant’s academic performance and/or notable extra-curricular non-academic achievements, and other pertinent documents as may be requested by PSGIAC;

ADDITIONAL REQUIREMENTS BASED ON CATEGORY:
- For Category C applicant, Certification from the school principal that he/she won the highest award/champion in any national or international competition and supported with certificates or citations, pictures of medals and trophies;
- For Category D applicant, Certification of CHO/MHO and recommendation from the CSWD/MSWD officer that the applicant is a Person With Disability (PWD);
- For Category E applicant, Certificate of Confirmation (COC) issued by the NCIP Regional/Provincial Office, proving that he/she is a member of the Indigenous People (IP) Community;
- For Category F applicant, Certificate of Confirmation (COC) issued by the National Commission on Muslim Filipinos (NCMF), proving that he/she applicant is Muslim;
- For Category G applicant, photocopy of his or her Solo Parent ID from the Municipal or City Social Welfare and Development Officer (present original; and
- For Category H applicant, a recommendation from his/her senior high school adviser, with recommending approval from the PTA president and the school principal, that the applicant is deserving of a scholarship because of extra-curricular skills and talent and he/she is known to be an industrious and persevering student with good moral character;

POST GRADUATE – JURIS DOCTOR/MEDICINE (Any Year Level)
- Thoroughly accomplished Application Form;
- One recent photograph (passport size);
- Application letter addressed to the Governor;
- Barangay Chairperson Certification that he/she is a bonafide resident of the barangay since birth/ for a period of not less than 1 year;
- Certified photocopy of Diploma, and 1st and 2nd Semester Transcript of Records of the school year last attended;
- Certificate of Enrollment/ Statement of Account;
- Certificate of Good Moral Character from the school last attended;
- Barangay Clearance that applicant has no derogatory records/ no complaint has been filed against him/her;
- Latest certified true copy of the applicant’s/ parents’/guardians’ BIR Income Tax Return, or if not available, a Barangay Certificate that the applicant’s family is a low income earner stating the estimated annual income;
- Certified True Copy of Birth Certificate (present original);
- Court Clearance that there is no pending case filed against the applicant; Police Clearance is a pre-requisite;
- Certificate of No Scholarship Grant/Scholarship Application from the school where the applicant is enrolled;
- Applicant’s or applicant’s parents’ Voter’s ID or Voter’s Certificate;
- Sketch Map of applicant’s home address;
- Documents commending the applicant’s academic performance and other pertinent documents as may be requested by PSGIAC.
